The recovery-failed popup returned after two days — the first diagnosis had repaired only the 18 rows whose base_url was written in plain sight, while most old sessions carried only a billing_base_url stamp inside model_config, treated as a launch route on resume and still inked with the dead address: of 175 sessions, 76 were stopped by the credential guard. After backup, 75 billing stamps were rewritten and one route to a delisted provider cleared; the gateway re-scan shows zero blocked. The deeper root lay in the code: the self-heal branch could never fire — that was why every endpoint move re-broke old sessions.
